A geriatric care manager is educated and experienced in fields related to care management, including registered nursing, social work, public health or psychology, with a specialized focus on issues related to aging and elder care.
General Supervision
Routinely perform supervisory visits to study caregivers’ documentation logs, medication administration, assess and adjust care plans as needed, and offer medical management.
Medical Management
Facilitate communication between doctor, patient and family by attending doctor’s appointments, ordering home health and medical supply, consulting with pharmacists, arranging transportation and more.
Resourceful
Providing local opportunities for social activities, access to preferred network of legal/financial professionals, research and secure entitlements, and maximize a patient’s health insurance for best care.
